Swab Left Inside Mum During Labour

A mum giving birth at Treliske has had a swab left inside her.
Bosses say they are investigating how it happened last month.
The incident is referred to internally as a "never event" because it should not happen under any circumstances.
It is believed to have happened during an assisted birth in the Royal Cornwall Hospital's delivery suite.
Medical Director Paul Upton said: “RCHT is constantly improving safety standards and learning from incidents to improve the quality and safety of our care. The Trust is currently investigating the incident to identify the causes and will ensure appropriate actions are taken.”2:04pm 4th October 2012
